Would these fit a men's size 12 shoe? Not sure what size 12 equates into boot sizes.

no you are going to need some large bindings for those boats...these will fit up to men's 10 or smaller or womens boots...

Do these have plates includes?

Yes, plates and screws. The discs are 4x4 pattern. I will have them over at t-line on wed. let me know if you want to take a look.
